Travelling With a GLP-1: The Storage Limits, Compared
How long each pen can be out of the fridge — 21, 28, 30 or 56 days depending which you have — plus what really forces you to bin it. One table, off the labels.

The only question that actually matters
Not "can I travel with it" — you can. The question is how long it can be out of the fridge, and the answer is different for every pen.
People get this wrong because they ask a friend on a different medicine. The numbers are not the same.
The limits, straight off the labels

Two molecules, four presentations, four different numbers: 21, 28, 30, 56. Two things to note. The Zepbound single-dose figure is a total, so time out of the fridge before the trip counts against it. And an opened Zepbound multi-dose vial or KwikPen is capped at 30 days at room temperature, 30 days from first use, or four weekly doses — whichever comes first1.
What forces you to bin it
The labels give one hard discard rule and two strong protections. Worth knowing which is which:
- Frozen — do not use it. That is an explicit label instruction, not a precaution, and it includes freezing against the back wall of a fridge or in an over-cold cooler2.
- Keep it out of the light, in the original carton until you inject. The labels instruct you to protect it from light and, for Zepbound, from heat — they do not attach a discard rule to a light exposure, but the carton requirement is the label's, not ours12.
- Do not exceed the stated temperature range. Neither label tells you to bin a pen after a single brief excursion above 30°C, so if it happens, ask a pharmacist rather than guessing in either direction.
- The Wegovy single-dose pen's 28 days only applies before the cap comes off2.
A car in summer, a windowsill, a beach bag and a checked hold are all plausible ways to exceed 30°C. That is the failure mode worth planning around, not the day count.
Packing it
- Carry it on. A hold can get cold enough to freeze and you cannot check on it.
- Original carton. Light protection is a label requirement, not a suggestion.
- A cool bag, not an ice pack against the pen. You are avoiding freezing as much as heat.
- Count the days you have already used. Especially on Zepbound, where 21 days is cumulative.
- Take one more dose than the trip needs, in case one gets compromised.
- Photograph the carton, including the lot number and expiry.
Time zones and your dose day
Weekly dosing is forgiving. Keep the same day and do not try to match the hour across zones — the interval matters far more than the clock.
If a dose falls mid-flight, take it before you leave or after you land; there is no need to do it on a plane.
If you do miss one, the windows also differ: for Wegovy, take it if the next scheduled dose is more than 2 days away, otherwise skip; for Zepbound, you have up to 4 days12.
Two things worth planning for
Titration and travel are a bad combination. A dose increase in the days before a trip means arriving in the window where gastrointestinal effects cluster. Ask to hold the step until you are home — a routine request.
And build in the boring one: dehydration is easier on a travel day, and gastrointestinal reactions that cause volume depletion are exactly what the labels tell clinicians to watch for2. If you cannot keep fluids down, that is a same-day call wherever you are — find out how to reach your provider from abroad before you need to.
Take a photo of the carton before you go. If you end up calling from a hotel, it answers the first three questions for you.
Frequently asked questions
How long can a GLP-1 be out of the fridge?
It depends which one, and the numbers are not the same. A Zepbound single-dose pen or vial can be kept below 30°C for up to 21 days in total, then must be discarded. A Wegovy single-dose pen or syringe can be stored at 8°C to 30°C for up to 28 days, in its original carton and before the cap is removed. A Wegovy FlexTouch pen, after first use, can be kept at 15°C to 30°C for up to 56 days.
Should I put it in checked luggage?
Carry it on. A hold can get cold enough to freeze the medicine, and anything frozen has to be thrown away regardless of how many days it has been out of the fridge. You also cannot check on it or move it if conditions change.
When do I have to throw it away?
Freezing is the clear one — the labels say do not use it if it has been frozen. Beyond that, discard when the day count for your specific presentation has run out: 21 days for a Zepbound single-dose pen or vial, 28 for a Wegovy single-dose pen before the cap comes off, 30 for a Zepbound multi-dose vial or KwikPen, 56 for a Wegovy FlexTouch after first use. The labels also tell you to keep it in the original carton and out of the light, though they do not attach a discard rule to a brief light or heat exposure — if that happens, ask a pharmacist rather than guessing.

What if I miss a dose while away?
The windows differ by medicine, and they are structured differently — Wegovy counts backward from your next dose, Zepbound counts forward from the one you missed. For Wegovy, take the missed dose if your next scheduled one is more than two days away, and skip it if it is sooner. For Zepbound you have up to four days, after which you skip it and resume on your usual day; the label also asks for at least 72 hours between any two doses.
